ARE YOU AT RISK FOR A SERIOUS FALL?

PLAY 20 QUESTIONS AND FIND OUT!

  1. Do you take smaller steps than you once did?
  2. Do you shuffle instead of picking up your  feet?
  3. Can you get up from or into a chair without using your arms at all?
  4. Can you get in and out of your shower or tub without slipping, tripping or bumping into things?
  5. Do you sit down hard - practically falling into your chair or onto the toilet seat?
  6. Can you get up from the floor using only your arms to help you?
  7. Do you have trouble getting up to or down from a curb without support?
  8. Have you tripped in the house or outside in the last  month or two? More than once?
  9. Have you fallen in the last year? More than once?
  10. Have you lost your balance in the past 30 days?
  11. Are you afraid of falling?
  12. Do you feel unsteady on your feet?
  13. Do you use a cane or walker for support?
  14. Do you have osteoporosis?
  15. Do you have a history of stroke, Parkinson's or a family history of falls?
  16. Do you have difficulty walking? (weakness, fatigue, pain)?
  17. Are you over 80?
  18. Do you take more than 4 medications for your heart or blood pressure or for depression?
  19. Do you have problems with circulation to or feeling in your feet?
  20. Do you have vision problems that have not been corrected?
